- PLEASE BE SURE TO COMPLETE YOUR MEDICAL FORMS PRIOR TO YOUR APPOINTMENT, IF YOU ARE UNABLE TO FILL OUT YOUR FORMS PRIOR TO YOUR APPOINTMENT YOU MUST ARRIVE 10 MINUTES EARLY PRIOR TO YOUR APPOINTMENT. (It is of utmost importance that your forms are filled out or you may risk not being seen for your appointment)

- PLEASE PREPARE FOR YOUR VISIT BY GIVING YOURSELF TIME TO FIND PARKING (There are parking lots on both sides of our business)

Deposit Policy
- All new patients to the practice or patients requiring a consultation are required to pay a $150 deposit fee in order to schedule an appointment
- Deposits made are non-refundable
- This deposit can be applied toward services rendered (This excludes Weight Loss Consult Fees)
- Failure to comply to our cancellation policy may result in a loss of your deposit
Cancellation Policy
(We value your time and we ask that you please respect our providers’ time as they are booked out weeks to months ahead and have a significant waiting list. Last-minute cancellations do not allow time to fill those gaps. We appreciate your understanding of our cancellation policy)
- We respectfully request at least 48 business hours notice if you need to cancel or reschedule your appointment.
- Any cancellations / reschedules made less than 48 business hours from your scheduled appointment day will result in a $150 late cancellation fee.
- If a patient no shows and/or late cancels 3 times, the patient will be discharged from the practice.
- Failure to comply to our cancellation policy may result in a loss of your deposit- Should you need to cancel, please do so at least 48 business hours in advance of your scheduled appointment, this excludes Saturday and Sunday.
- If you cancel with less than 48 hours’ notice, your credit card on file will be charged a cancellation fee.
- If you NO SHOW for your scheduled appointment without providing notification, the service will be considered rendered and you will either be charged the full amount of the service or the cancellation fee, whichever is greater.
- Appointments scheduled for Monday must be cancelled/rescheduled by 9 am Thursday in order to avoid cancellation fees.
- Should you arrive late (more than 10 minutes) you are considered a late cancellation/ no show, and the full amount of the service or the cancellation fee (whichever is greater) will be charged to your credit card on file.
- If you scheduled multiple services, and cancel or no-show with less than the required 48 hour notice you will be charged a late cancellation, no show fee per each service.
Late Policy
- New patients are advised to arrive 10 minutes early to their appointment time so you do not miss your appointment and you have enough time to find parking.
- Should you arrive late (more than 10 minutes) you are considered a late cancellation/ no show, and the full amount of the service or the cancellation fee (whichever is greater) will be charged to your credit card on file.
Refund Policy
We do not offer refunds on any services rendered.
Aesthetic results are vary from person to person and while we do our best to achieve the desired outcome, it cannot be guaranteed. Clients are responsible for additional treatments needed to achieve desired results.
Retail
We do not offer refunds on products purchased. All sales are final.
We do not offer refunds on Gift Certificate purchases.
Children Policy
At KÜR we love children, however, we ask that you please make child care arrangements prior to your appointment. This ensures the safety of your children as well as a relaxing environment for all clients.
Pet Policy
At KÜR we love animals, yet we ask that you please leave your pet at home during your visit to KÜR.
