Appointment Instructions
We Can't Wait to Welcome You!
To ensure a smooth experience, please review the following information before your appointment: Complete your medical forms prior to your visit by clicking our PATIENT PORTAL. If you are unable to do so, please arrive at least 10 minutes early to complete them on-site. It’s important that forms are filled out ahead of time to avoid the risk of your appointment being rescheduled.
Contact us if you experience any technical issues with the forms. A valid government-issued ID is required for your appointment. Unfortunately, we cannot proceed without proper identification. Plan to arrive early to allow time for parking. There are parking lots next to our building, and street parking is also available. You can pay for street parking using PARKMOBILE.

Deposit Policy
- All new patients to the practice or patients requiring a consultation are required to pay a $150 deposit fee in order to schedule an appointment. (deposits made are non-refundable).
- Your deposit fee can be applied to your consultation, treatment, or product purchase.
- If you decide against any further treatments after your completed consultation, the $150 consult cost will then go toward your provider’s time.
- Deposits are required furthermore for procedures such as injectables, skin treatments, and any other consultations.
Cancellation / No Show Policy
- We value your time and we ask that you please respect our providers’ time (We appreciate your understanding of our cancellation policy).
- The only exception to this cancellation policy is in the event of a serious or illness or emergency.
- We respectfully request at least 48 business hours notice if you need to cancel or reschedule your appointment.
- Any cancellations / reschedules made less than 48 business hours from your scheduled appointment day will result in a $150 cancellation fee. If you cancel with less than 48 hours’ notice, your credit card on file will be charged a cancellation fee.
- If a patient no shows or breaks the cancellation policy 3 times, the patient will be discharged from the practice.
- Failure to comply to our cancellation policy may result in a loss of your deposit- Should you need to cancel, please do so at least 48 business hours in advance of your scheduled appointment, this excludes Saturday and Sunday.
- Appointments scheduled for Monday must be cancelled/rescheduled by 9 am Thursday in order to avoid cancellation fees.
- If you scheduled multiple services, and cancel or no-show with less than the required 48 hour notice you will be charged a late cancellation, no show fee for each service.
Late Policy
- We allow a 10 minute grace period, & you may be asked to reschedule.
- Should you arrive late (more than 10 minutes) you are considered a late cancellation/ no show, and a $150 fee will be charged to your credit card on file.
Refund / Exchange Policy
- We do not offer refunds or exchanges on any services rendered, all sales are final.
- Aesthetic results may differ from person to person. At KÜR we always strive to understand your personal goals and achieve your desired outcome, but it cannot always be guaranteed. Clients are responsible for all additional treatments needed to achieve desired results.
- We do not offer refunds on retail skincare purchased. Damaged products may be exchanged within 14 days for the same product only.
- We do not offer refunds on Gift Card purchases.
Children Policy
- At KÜR we love children, however, we ask that you please make child care arrangements prior to your appointment as we do not allow children due to safety concerns & the nature of the environment to ensure safety & comfort for all of our patients.
Pet Policy
- At KÜR we absolutely love all animals (in fact all of our staff members have pets), but unfortunately we do not allow animals at our practice.
- We truly appreciate your cooperation and consideration.
